Formula Rationale N-acetylcysteine (NAC) has been studied in various clinical contexts over twenty years. NAC is a mucolytic nutrient that has antioxidant, hepatoprotective, and detoxifying effects. NAC increases cellular glutathione levels, which has a host of beneficial effects. Furthermore, it is used in hospital settings to treat poisonings such as Tylenol overdose and mushroom toxicity.
A human study of 262, primarily elderly, individuals indicates that NAC may decrease influenza symptoms. In the study, 25 percent of virus-infected subjects who received NAC treatment developed symptoms, whereas 79 percent in the placebo group developed symptoms. NAC has reportedly been able to enhance the effectiveness of alpha-interferon therapy in patients with hepatitis C. Glutathione levels increased along with improvements in several other disease parameters that were being measured. In the treatment of HIV infection in HIV-infected patients, therapy with NAC results in enhanced immunological activity, including improved natural killer (NK) cell and T-cell functions. NAC is a potent mucolytic agent. When utilized as a 10% solution with a bronchodilator by pressure machine, clearance of mucous secretions can be achieved in asthmatic subjects without inducing bronchospasm. A meta-analysis of ten clinical trials revealed that prolonged treatment with oral NAC provides substantial benefit in the treatment of chronic bronchitis. Dosages ranged from 400mg to 1,200mg daily, and the duration of the studies ranged from three to six months. Chronic bronchitis occurs frequently in smokers, and NAC reportedly reduces the incidence of recurrent infections in these individuals. At 600mg per day, NAC has proven to be beneficial in the treatment of chronic obstructive pulmonary disease due to its antioxidant and mucolytic properties, and its ability to reduce hydrogen peroxide induced damage.
NAC acts as a chelating agent in the treatment of acute heavy metal poisoning, both as an agent capable of protecting the liver and kidney from damage and as an intervention to enhance elimination of the metals. For example, NAC helps detoxify and protect against methyl mercury, which is a potent neurotoxin and one of the most dangerous environmental pollutants. NAC also detoxifies arsenic, chromium, and boron. NAC lessens the cardiotoxicity associated with doxorubicin (Adriamycin), and causes greater than a 50 percent reduction in the incidence of nausea and vomiting in patients receiving cyclophosphamide, epirubicin, and carboplatin. Elevated lipoprotein(a) levels have been associated with an increased risk for atherosclerosis and thrombotic disease. Patients receiving NAC in dosages of 2 gm/day for four weeks, and then 4 gm/day for the next four weeks, experienced nearly a 70 percent reduction in Lp(a) levels. In patients with chronic kidney disease,inflammation and oxidative stress are cardiovascular risk factors. NAC has been shown to reduce the number of cardiovascular events in hemodialysis patients.
Results from animal experiments suggest that NAC may help inhibit the development of retinopathy in individuals with diabetes. N-acetylcysteine has been shown to reduce the symptoms of both schizophrenia and bipolar disorder in two placebo-controlled trials conducted at Melbourne University. It is thought to act via modulation of NMDA glutamate receptors. NAC has been shown to reduce cravings associated with chronic cocaine use in a study conducted at the Medical University of South Carolina., It may also protect against autism, glucose toxicity, OCD, nail biting, compulsive hair pulling, PCOS, Sjogren’s syndrome. It has been studied by the US Army and Navy and may help to reduce noise induced hearing loss.
